If we haven't yet had the pleasure of meeting you and your pet, please fill-out the Client Information section in the green area below and  submit prior to your appointment. Thank you. 

It is also a good idea to give us a head start in preparing for your visit by sending us your pet's medical history ahead of time, you can email or fax to [email protected] or (Fax: 360-659-5524). If your pet has a chronic medical condition or is on prescription medication, we ask that you send previous medical history (2) days prior to your pets appointment, Thank you.
